This main focus of this role is to provide safe discharge for the older population, NDIS clients, homelessness, indigenous and torres straight islandanders, victims of domestic/family violence and older people experiencing any form of elder abuse. With in this role collaborating closely with the multidisciplinary EMAT team is vital for the best possible outcome for some of our most vulnerable members of our community. This include such specialty as social workers, physiotherapist, GEDI and GITH team, occupational therapist and dietitians. This role was changeling but extremely rewarding and I am still currently filling in annual leave and sick leave.
