• Contribute to collaborative casework by maintaining records of progress notes on a weekly and monthly basis, and provide reports to allied health professionals, family members, and guardians.
• Facilitate liaison and support with professional groups and community members to address key participant issues, including behavioural, mental and emotional concerns, family breakdowns, employment, skill upgrades, and more.
• Prepare budget reports for individual participants in consultation with the Client Service Coordinator and obtain approvals from NDIS and other relevant stakeholders.
• Develop a clear understanding of each participant's needs, document them through case formulation, and provide regular staff training.
• Organise appointments for participants and ensure their attendance.
• Act as the primary contact for clients and their families, providing collaborative holistic support based on their strengths, needs, goals, and aspirations within NDIS guidelines.
• Handle rostering and scheduling of team members.
• Manage day-to-day placement operations and continually seek improvements in existing systems.
• Collaborate with the Client Service Coordinator to conduct necessary training for team members.
• Conduct structured risk assessments in accordance with company policy.
• Assist with the training and induction of both current and new staff members.
• Perform daily monitoring of Progress Notes and other documentation.
• Support the Client Service Coordinator in preparing the Monthly Progress Report.
• Provide essential emotional support to special needs youth.
• Encourage youth clients to develop their own activities and leadership skills through group discussions.
• Manage and lead activities that foster growth in mental, emotional, and educational domains.
• Mentor and guide youth clients in DCP to develop skills for success in the community.
• Offer guidance and advice to youth, assisting them in navigating various personal and social challenges.
• Organise and facilitate recreational activities to involve youth in positive team-building experiences.
• Conduct one-on-one observations of youth to ensure their safety and that of staff during crisis situations, including suicide, mental health, and medical watches.
• Serve as a positive role model to promote Positive Youth Development (PYD).
