Professional Responsibilities:
Pharmaceutical Care & Medication Management:
Ensure the appropriate use of medications by evaluating therapy, including dosage, dosage forms, regimen, route, and potential drug interactions.
Perform thorough medication reconciliation, maintaining updated patient medication profiles with demographics, diagnosis, allergies, body weight, and current prescriptions.
Review and verify medication orders for accuracy and appropriateness, including cytotoxic reconstitution, IV admixture, total parenteral nutrition (TPN), and unit-dose medications.
Patient Safety & Quality Assurance:
Conduct double-checking, filling, and dispensing of medications with accuracy and compliance to safety protocols.
Identify, monitor, document, and report adverse drug reactions and medication errors to ensure patient safety.
Participate in nursing unit and clinic inspections, ensuring adherence to pharmaceutical and regulatory standards.
Clinical & Advisory Role:
Collaborate with prescribers to clarify medication orders, ensuring optimal patient outcomes and proper documentation.
Provide evidence-based drug information and consultation to physicians, hospital staff, and patients.
Document and track clinical pharmacy interventions to support continuous quality improvement.
Supervisory & Leadership Duties:
Lead and supervise pharmacy technicians and trainees, ensuring adherence to best practices and institutional guidelines.
Manage and oversee pharmacy operations in outpatient, inpatient, oncology, and ambulatory care settings.
Direct the pharmacy materials management process, overseeing refill services, emergency room medication services, and call center operations.
