Roles And Responsibilities

  • Engage with participants and their key decision-makers to ensure they understand their NDIS plan and house-related support.
  • Liaise with a variety of service providers on behalf of the participants.
  • Provide participants with sufficient information to enable them to make informed decisions relevant to their NDIS goals.
  • Connect participants with providers and assist them to implement their plans.
  • Build participant capacity to achieve outcomes.
  • Arrange appropriate meetings with clients and their families or nominees.
  • Coordinate plan implementation and monitor agreed to fund for support.
  • Understand the eligibility requirements for NDIS home and living supports which will include Various housing options.
  • Build and maintain relationships with community organization and NDIS partners
  • Promote awareness of housing options and supports available to NDIS clients within the community.
  • Work with participants to address the barriers and crisis/conflict as required.
  • Prepare and submit reports as required in a timely manner.
  • Record billable hours and case notes within 24 hours
  • Apply knowledge of incorporating individual choices, goals, and aspirations into service delivery
  • Monitored services to assure they are being delivered as planned and provide regular updates to the stakeholders and families regarding client’s improvement.
