The key tasks I performed there was:

  • To attend the personal needs of the resident including all activities of daily living, social emotional support in accordance with the care plan and at the direction of the supervisor care leader/ Duty nurse.
  • To protect the rights of residents and ensure care provided maintains resident's dignity and privacy.
  • To support resident's choice and decision making.
  • To follow correct Manual Handling Procedure.
  • To report and document any changes in a resident's health and well-being.
  • To recognize health and environmental hazards and reports damages, faults, and problems, using appropriate incident reporting.

1ST Year of Nursing placement at Calvary McDonald (4 weeks)

0ctober 2022- November 2022

My duties were:

  • Provided support and reassurance to residents with dementia and other mental health conditions
  • Attending promptly and respectfully to residents' activities of daily living (ADL's) needs
  • Safely lifted and moved patients using the correct manual handling procedures including the stand lifter and sling lifter.
  • Providing emotional and psychological support to the patient.
  • Vital signs and daily assessment of clients (Including a head-to-toe assessment, falls risk assessment, and pain assessments).
  • Gained experience in diabetic care including blood Glucose monitoring and observation of administration of regular ordered insulin and sliding scale insulin. Diabetic assessment and diabetic education for the client.
  • Assist the EN With Medication administration, utilizing the eight rights of medication administration, including eye drops and puffers and spacers and nebulizer.
  • Assessment and planning daily for wound assessments including simple wound dressings of skin tears, pressure injuries and venous ulcers. Understanding of primary and secondary wound dressings
  • Falls risk assessments including recognizing and responding to a client who has had a fall and monitoring their condition as per the facilities guidelines (neurological assessment and follow up for the fall).
  • Shadowing and working with the team leader EN and RN and providing support and assistance with the tasks they are required on a shift.
  • Listen to and participated in daily handovers handover with the team.
  • Utilized the computer (ICare) for documentation and update of care plans, document vital signs, pain assessments, wound assessment chart, BGLs and completed a progress note.

Completed 2nd Year of Nursing placement at TQEH (4 weeks)

May 2023- June 2023

  • Provide care that demonstrate awareness of relevant legislation and CALNH policies, is ethical and is respectful of individual needs and culture and is supported by evidence-based practice whilst under the supervision of nursing team member
  • Perform and document observations, informing the registered nurse of all observations performed in a timely manner.
  • Able to recognize and respond appropriately to clinical deterioration. Immediately communicate observations to supervising RN when observations are in yellow zone and continue to support patient observations hourly under supervision of Registered Nurse until resolved.

  • Administered IV, IM and subcutaneous medication by following 8 right of medication under the supervision of registered nurse.

  • Managed a workload of 3-4 patients per shift, consulting senior nurses as needed to prioritize tasks and address complex patient needs.
